The Hand of Wisdom and Action is a unique glove that can drop from defeating Xesht, We That Are One, the pinnacle Breach boss in the game. To encounter Xesht, players must enter the Twisted Domain, accessible through the Realmgate on the Atlas, which can be activated using a Breachstone. The Realmgate is conveniently located near the starting point on the Atlas, marked by a home icon that you can click to focus the camera on this area.
Alternatively, HoWA can be purchased on the Path of Exile 2 official trading website. However, even the lowest rolled versions of these gloves can command prices in the range of multiple divines due to their rarity and value.
How to Get a Breachstone in PoE 2
There are two primary methods to obtain a 
Breachstone in Path of Exile 2:

Can You Use Orb of Chance to Get Hand of Wisdom and Action
Unfortunately, no. The Hand of Wisdom and Action is exclusively a drop from Xesht, We That Are One, and does not appear on the regular loot table. This means you cannot use an 
Orb of Chance on White Furtive Wraps to try and obtain it, unlike items such as Astramentis, 
Headhunter, or 
Polcirkeln.
