Navigating the Kentucky BioTech Facility
The mission begins with Case and Marshall entering a Kentucky BioTech facility filled with toxic gas, necessitating gas masks. An elevator malfunction leads to a fall and gas mask breakage, triggering hallucinations. After the cutscene, the player regains control.
Locating the Security Desk
The immediate objective is to find a locked red-lit door. Use a hatchet found on a mannequin to break it open. Proceed through the hallway, up the stairs, and to a central elevator. Activating the elevator unleashes hallucinatory zombie mannequins; eliminate them with the hatchet. A phone call from the circular desk directs you to the biotechnology room, requiring four director's cards (Red, Green, Blue, Yellow). A map points to the Yellow card's location.
Acquiring the Yellow Card and Grappling Hook
From the security console, follow the map to a yellow staircase leading to the Director's office. Solve the computer puzzle ("Access" and "Lift") to access the A.C.R. room. More axe-wielding zombies appear.
After eliminating them, approach the mannequin holding the yellow card. This triggers a transformation into an abomination. Before engaging, gather armor plates, weapons, and critically, the grappling hook (shown below) from the surrounding area.
Use tactical explosives (C4 or grenades) to quickly eliminate the abomination and its zombie horde. Retrieve the Yellow Card from the abomination's remains.
Securing the Green Card
Use the grappling hook to ascend to the main facility. Next, obtain the Green Card from the Administration Facility. Grapple to the Administration Facility from the security desk. Answer the ringing phone. Locate and place four documents in the file display area. Mannequins will pursue; maintain distance by sprinting. Document locations: a desk in the corner, near a round table, on a small central table near a noticeboard, and in the cafe near a sink. After placing the documents, grapple the attacking red mannequin repeatedly until it transforms into a Mangler Zombie. Defeat the Mangler to obtain the Green Card.
Obtaining the Blue Card
Grapple across from the Administration Wing balcony to the Joint Projects Facility. Answer the phone. Locate camera stands surrounding a glass chamber containing the Blue Card. Eliminate the spawned Mimic (which can disappear; shoot moving objects to reveal it). Collect the Blue Card.
Retrieving the Red Card
From the security desk, go to the East Wing, following red carpets to stairs. In the resulting room, interact with the console to reveal the Red Card held by a Mangler. Use the grappling hook to reach the upper area, jump into the pool, and swim through a red tunnel. Climb ladders, eliminate zombies, and unlock a door using a blacklight-revealed code. Start a timer and turn all drain switches within 25 seconds. The final switch requires the grappling hook. After draining the water, pursue the escaped Mangler and defeat it and its horde to get the Red Card.
Confronting the Disciple
Return to the security desk and insert all four cards. Use the elevator, eliminating any pursuing zombies. Answer the red phone in the BioTech room, fight the zombies and the Disciple. A final cinematic reveals the events as a hallucination.
